If your event has NOT been approved, please email Cody Greenberg for approval PRIOR to filling out this form.
This person will be responsible for the initial design/logistics back & forth with Melanie Appelbaum
This person will receive as close to a final version as possible and is responsible for making sure that everything is factually and logistically correct.  Design changes will not be made at this point in the process unless there is something glaring.
